Communication understood in context

Understand the whole child.
Build from the core.

ModusCore connects evaluation, developmental foundations, and clinical planning so the people supporting a child can move forward from the same clear understanding.

When we do not yet know the cause, we stay curious.

We gather evidence before turning an observation into a conclusion.

The ModusCore approach

Not a snapshot.
A structured way of seeing.

Communication is shaped by language, cognition, sensory regulation, motor demands, attention, experience, relationships, and environment. ModusCore uses a bottom-up, evidence-seeking process to understand how those systems may be affecting access and performance.

  1. 01

    Look beneath the surface

A behavior, missed response, or low score is information—not an explanation by itself.

  2. 02

    Find the access point

Determine how the child best understands, responds, participates, and shows what they know.

  3. 03

    Test the possibilities

Use multiple sources and changing supports to separate emerging capacity from current performance.

  4. 04

    Build the next layer

Sequence recommendations from foundational needs toward meaningful, functional outcomes.

The bottom-up theory

Language from the foundation up.

The ModusCore Pyramid Language Model does not begin by treating the most visible communication difficulty. It first identifies the foundational system that may be limiting access, language, communication, or learning.

Look deeper. Treat smarter. Build communication from the foundation up.

One framework, three connected environments

Understanding should travel with the child.

Each part of ModusCore stands on its own. Together, they reduce the fragmentation that happens when evaluation, school, and treatment use different information and different priorities.

01

Evaluation

Find the most accurate starting point.

Independent speech-language evaluations combine records, interviews, observation, direct assessment, and carefully selected informal probes. The goal is not simply a score—it is a defensible picture of communication, access, and next steps.

  • Language and academic communication
  • Complex communication and access
  • Educational planning and second opinions
02

Teacher Companion

Turn findings into usable support.

Clear priorities become practical supports. The Teacher Companion keeps communication goals connected to instructional demands, environmental access, and what educators observe across the school day.

  • Practical instructional strategies
  • Observable communication targets
  • Shared language across team members
03

Homecare Clinician

Turn findings into a coherent treatment path.

Clinician-facing planning organizes targets in a developmentally appropriate sequence, links each activity to a rationale, and keeps therapy materials and documentation aligned with functional communication.

  • Hierarchical treatment planning
  • Purpose-built session materials
  • Rationale and documentation alignment

Why ModusCore exists

“A child’s performance should never be mistaken for the full limit of their ability.”

Built from more than 30 years of speech-language practice.

ModusCore was founded by Tammy Nesmith, MS/SLP/TSHH, a New York State-licensed speech-language pathologist whose experience spans school-based practice, early intervention, complex communication, and speech-language evaluation.

The framework grew from a recurring problem: important information about a child is often separated across reports, observations, caregivers, and clinical sessions. ModusCore brings that information into one disciplined process—without forcing every child into the same testing path or the same explanation.

ModusCore operates as an independent private practice.
